It is time for you to write something about yourselves. For that purpose we have created a file. You have to open it on a computer where the programme Adobe Reader is installer. You fill in the form, print it out and give both pages to your teacher. Please note that everyone will be able to read the first page. The second page is only for the teachers. About smoking: The important thing for us to know, is whether you or your parents smoke inside the house. If that is the case, you must write on page 2. If not, it isn't that important to us.

The Danish students have to do this as soon as possible.
